I was referred to your practice by my doctor, how long will it be until I am called for an appointment? The referring providers fax over the notes and request for an appointment. Once we receive the fax, it takes approximately five business days for a return call. Please save our number in your phone, 423-698-8981 so that when we call you will recognize the number and we can schedule your visit. Delays in booking often from call phone tag.
I need to reschedule an existing appointment, what should I do? Please call our office at 423-698-8981 to reschedule. Our office is open from 8:30 AM until 5:00 PM Monday thru Thursday and 8:30 AM until 4:00 PM on Friday. We are closed for lunch each day from 12:00 PM until 1:00 PM. Our answering service will not accept reschedule messages.
I am a new patient and would like to be seen by your practice, what should I do? Please check out our Insurance Plan list to make sure that we are in network with your insurance plan. If we do not accept your plan, you will need to call your insurance company to find a specialist that is in your plan. In some instances, we offer a self-pay rate if we are not in your network.
I have questions about the bill I received from the practice, what number should I call? Our billing is handled by Specialty Billing and their direct number is 423-760-4875. Their office can be reached during normal business hours Monday-Friday 8:30 to 5:00.
Does your practice complete FMLA paperwork? Yes, we will complete FMLA paperwork if the patient portion is completely filled out by the patient or guardian before dropping off. There is a $25.00 fee that must be paid with dropping off the paperwork to be completed. These forms can be mailed in with a check to our main office at 901 Riverfront Parkway, Suite 300, Chattanooga, TN 37402 or hand delivered and payment can be made at time of drop-off. Please do not fax the forms into our office as they will not be completed via that delivery mechanism.
If I have a high-deductible health plan or have not met my deductible and no copay, will you bill me after my insurance pays for office visits? We will bill your insurance if we participate with that plan; however, if you have not met your deductible you will be required to pay at least a $200 deposit on the day of your visit for all visits for patients 18 and over and a $50 visit for patients under the age of 18. If your insurance pays the bill and it results in a credit on your account, we will send you a refund back to the method of payment you used for your deposit.
If I have a co-pay, will you bill me after my insurance pays for office visits? Co-payments are an agreement between your plan for your responsibility outside of any deductible you may have. All co-payments are due at the time of check-in. If you do not have the co-payment at the time of your visit, you will need to reschedule.
